[The main topic is about adult students returning to college, while the idea or angle about the topic is that there are reasons for the higher percentage of adults returning to college. A thesis sentence's angle should NOT be:

  • Too broad. For example, the following thesis really doesn't pinpoint a specific insight about the topic:  Adult students returning to college have a hard time. In what ways do adult students have a hard time? You'd need to identify a more specific insight in this angle.
  • Too narrow a statement of fact. For example, the following thesis really cannot be developed into a full essay because the angle doesn't contain the writer's own thoughts or insights about the subject:  Adult students returning to college read an average of 7.5 books per term. A reader may respond by saying, "So what?" A narrow statement of fact does not contain your own personal analysis, argument, or interpretation of the topic--that all-important angle which a thesis must have.
  • An announcement. For example, the following really is not a thesis at all because it lacks an angle that gives the writer's own insight into the topic: My topic is the adult student returning to college. Again, "So what?" What's the reader's idea here?

Once you create a working thesis, you should assess it to make sure that it fulfills thesis characteristics.  Make sure it has a clear topic (indication of what the thesis is about) and angle (what your own ideas are about the topic, i.e. what you are trying to prove). Make sure that the angle is not too broad, too narrow, a statement of fact, or an announcement. Work with the angle to make it indicate the order of your support, if you choose to do that for yourself or for your reading audience. And realize that the thesis is a working thesis until you finalize the essay (it's okay to revise the thesis as you go along, just as long as you retain important thesis characteristics.)

Literature Review of Anatomical Variations: Clinical Significance, Identification Approach, and Teaching Strategies

This article is a comprehensive literature review on anatomical variations, shedding some light on their clinical significance, identification approaches, and teaching strategies. Anatomical variation is a normal presentation of body structure with morphological features different from those that have been classically described in several anatomy textbooks. Under normal circumstances, it has no impact on the function of the structure. However, it may influence different aspects of clinical practice. As a result, accurate knowledge of common anatomical variations in the treated area may improve clinical practice outcomes. On the other hand, anatomical variations are usually identified during routine dissection and clinical practice, including preoperative imaging and surgical procedures. Additionally, scientific research, such as observational studies using cadaveric dissection, medical images, and evidence-based anatomy, are effective approaches to identify anatomical variations. With regard to the teaching of anatomical variations, cadaveric dissection is the most commonly used and recommended learning tool for teaching anatomy and relevant variations in medical schools. However, the literature emphasizes introducing anatomical variations in the clinical phase of medical curricula and postgraduate training of surgical and radiological programs. The current study suggests creating a registry of anatomical variations encountered during practice that may enhance best care and prevent any confusion about those variations. In addition, there is a need to conduct more educational studies to research the best learning strategies for teaching and assessing anatomical variations in the medical curricula.

Introduction and background

Choosing which running shoes to buy is evidence that our feet are not all the same. Therefore, normal does not always mean the same, especially regarding our bodies. Anatomical variation is a normal presentation of body structure with morphological features different from those described in the literature. It involves many types and forms of variations, including muscle structure, attachments, and innervation, ligament attachments and morphology, blood vessel branching, nerve patterns and positions, and bone morphology and accessory bones [ 1 - 7 ]. It has been found that some of these anatomical variations are closely related to ethnic groups [ 8 ].

Under normal circumstances, anatomical variations do not affect the function of the organ. However, clinically, anatomical variations play a significant role in diagnosis and therapeutic procedures in which they may require more attention or specific arrangements [ 9 ]. Anatomical variations can be closely linked to race, environmental adaptation, geographical region, and exposure to chemicals or radiation. Thus, identifying these variations will reflect positively on the clinical outcomes by minimizing any confusion or malpractice.

Identification of anatomical variations usually occurs during routine dissection and imaging investigations or surgical procedures. A study found that experienced clinicians reported seeing variations in their practice on a monthly (39%), weekly (25%), and daily (21%) basis [ 10 ]. It has been reported that ignorance of these variations was responsible for 10% of malpractice cases [ 11 ]. Therefore, the current review article will discuss the anatomical variations, focusing on their significance in clinical practice, approaches to identifying them, and how to teach them in medical schools’ curricula.

Significance of anatomic variations in clinical practice

The influence of anatomical variations on different aspects of clinical practice can be categorized into the predisposition to illness, symptomatology, clinical examinations, and treatment of patients, including surgery [ 12 ]. Previous studies found that individuals with left coronary artery dominance have a higher risk of in-hospital mortality and myocardial reinfarction than those with right coronary artery dominance [ 13 ]. The anatomy of the vermiform appendix also showed variable positions, including pelvic (56%), subcecal (19%), retroileal (13%), retrocecal (7%), ectopic (4%), and pre-ileal (2%) [ 14 ]. These variations may mislead the diagnosis in patients with acute appendicitis and may result in severe complications, such as perforation and sepsis.

A large body habitus can produce low-quality images in clinical examinations, affecting the accuracy of clinical interpretations of the images [ 15 ]. Another rare anomaly that can be seen during the clinical investigation of the chest is the azygos lobe of the lung [ 16 , 17 ]. It was seen in 0.4% of chest radiograms and may be confused clinically with a bulla or abscess. The routine clinical practice of measuring blood pressure may also interfere with the higher division of the brachial artery, which can occur in 20% of cases [ 18 , 19 ]. Arterial variation is common in the upper extremities. Therefore, accurate knowledge of these variations may help prevent limb complications such as injury, thrombosis, gangrene, or even amputation of limbs [ 20 ].

Furthermore, a safe laparoscopic cholecystectomy requires accurate knowledge of the cystic artery anatomy variations [ 21 ]. Originally, the cystic artery is a branch of the right hepatic artery, but it may also arise from other arteries, including the left hepatic artery, the hepatic artery proper, the common hepatic artery, the superior pancreaticoduodenal artery, the superior mesenteric arteries, or even the gastroduodenal artery [ 22 ].

The pectoralis minor also shows variable anatomy that is worth considering in clinical practice. The pectoralis minor is a significant anatomical landmark on the chest wall providing access to many related structures in the clavipectoral and axial regions [ 23 ]. However, the muscle shows several variations, including the origin and insertion of the attachment sites [ 24 , 25 ], muscle length [ 26 ], vasculature [ 27 ], and innervation [ 28 ]. Hence, knowing the anatomical details of the pectoralis minor is essential for many clinical applications and surgical procedures such as pectoralis minor release and nerve block [ 29 , 30 ].

In an attempt to increase awareness of anatomical variations in general surgery, Kowalczyk and Majewski discussed the variable anatomy that may influence the outcomes of common surgical procedures [ 31 ]. The listed surgeries included cholecystectomy, inguinal hernia repair, hepatobiliary surgery, spleen, colon, thyroid, breast, axillary, pancreatic, and gastric surgery. They recommended closing this gap, usually cited as a technical error in surgical injuries, through introducing dissection courses and using preoperative imaging techniques. Therefore, accurate knowledge of the anatomical variations may improve the clinical practice outcomes.

Approaches to identifying anatomical variations

Practitioners and anatomical scientists play a significant role in identifying anatomical variations. The typical traditional approach to identifying anatomical variations was through cadaveric dissection [ 12 ]. In contrast, some of the variations were identified in clinical practice during physical examinations, imaging investigations, and surgical procedures. In addition, the literature showed that scientific research, including observational studies and evidence-based anatomy, boosted identifying any anatomical variations.

The literature review showed several studies that reported rare cases of anatomical variations identified during routine laboratory dissections or surgical procedures. Benes and Kachlik [ 32 ] reported two cases of variable branching of the musculocutaneous and median nerves found during a routine dissection course. Kim et al. [ 33 ] also presented a novel variation identified during routine dissection in which the posterior division of the femoral nerve was penetrated by the ascending branch of the lateral circumflex femoral artery. Preoperative medical scans helped Vigneshwaran and his colleagues [ 34 ] to identify double variations of a horseshoe kidney and an accessory renal artery in a challenging case of low rectal cancer planned for laparoscopic resection. In addition, during preoperative diagnosis of a living liver donor, Gündoğdu and Kebapçı [ 35 ] observed an accessory right hepatic artery originating from the dorsal pancreatic artery and a middle hepatic artery arising from the pancreaticoduodenal artery. These preoperative observations of anatomical variations guided the surgeons to safe surgical resection of the cancer, and they searched for a new liver donor [ 34 , 35 ].

However, more significant results of anatomical variations are commonly reported through observational studies that inspect larger numbers of cadavers and medical images and outline their implications in health practice. A recent study assessed the challenges in using the posterior inferior cerebellar artery to the anterior inferior cerebellar artery bypass in 10 cadavers [ 36 ]. The anatomy of the two arteries was evaluated. Both arteries showed morphological and parametric variations, which made the bypass impossible. Another study examined the morphological anatomy of the sphenoid sinus using computerized tomography images of 320 adult patients [ 37 ]. The results revealed various pneumatization types, variable lateral extensions, multiple inter-sphenoid septa, and protrusion of the internal carotid artery into the sphenoid sinus. Identifying such variations is essential prior to any skull base surgery using the endoscopic endonasal transsphenoidal approach.

To enhance the findings of reported anatomical variations, both Yammine [ 38 ] and Henery et al. [ 39 ] introduced a new concept in the field: evidence-based anatomy. Evidence-based anatomy applies systematic review and meta-analysis principles to appraise and synthesize previous anatomical findings to generate a large, pooled sample size that is more likely to be accurate and reflect true population statistics and associations. Asghar et al. [ 25 ] published a systematic review and meta-analysis study about the prevalence of anomalous insertion of the pectoralis minor that included 4,146 shoulders pooled from 25 studies. Taterra et al. [ 40 ] also discussed the prevalence and anatomy of parathyroid glands in a meta-analysis study that included 7,005 patients and 23,519 parathyroid glands from 26 studies.

Nevertheless, a high clinical skill level is required to recognize and deal with these variations in clinical practice [ 12 ]. Thus, the current paper suggests creating a health informatics database to collect the common anatomical variations experienced in clinical practice. With the aid of information technology, the surgery and medical imaging units should gather any observed anatomical variations presented during clinical encounters. Those data should be compiled into a registry that includes the description of variations, demographic data, and the anticipated effects on routine clinical practice. Creating such a database will increase awareness of these variations, specifically to new resident doctors or medical interns that have joined the healthcare organization. The database of anatomical variations will support national health records by identifying common anatomical variations among ethnic groups. For example, the supernumerary heads of the biceps brachii muscle were reported commonly in South Indians [ 8 ]. Therefore, the registry can provide a benchmark and pattern identification and an opportunity to assist in developing standard care practices.

Teaching strategies of anatomical variations

The literature highlighted the need to increase the awareness of anatomical variations among medical students and clinical practitioners [ 9 , 12 , 41 , 42 ]. This call came as a number of medical schools are moving away from dissection and shifting towards plastic anatomical models and digital software [ 12 , 41 ]. Meanwhile, many medical schools teach anatomy to first-year medical students as typically described in atlas books, overlooking anatomical variations. Nevertheless, the literature shows a shortage in studies researching appropriate learning strategies for teaching the anatomical variations to junior medical students.

Previous studies emphasized that cadaver dissection is ideal for teaching anatomy [ 43 , 44 , 45 ]. Usually, students are assigned to dissect in groups of 6-8 students under the supervision of experts who are responsible for explaining concepts, such as anatomical variations, and their clinical application to students [ 43 ]. The long dissection hours, which may extend to 270 hours a year, give medical students the opportunity to explore and discuss with their peers many anatomical variations.

On the other hand, Kiss [ 44 ] shortlisted nine anatomical variations worth teaching to medical students, including variations in the circle of Willis, a missing dorsalis pedis artery, an internal carotid artery pseudoaneurysm inside the sphenoidal sinus, cervical ribs, a linguofacial trunk, arteria lusoria, variations of the coronary artery branches, variations of the liver anatomy, and variations of the cecum and appendix. However, students are commonly assigned to one cadaveric specimen, which decreases the chance to observe the listed variations. Thus, Sprunger [ 45 ] found that a cadaver reassignment system facilitates students’ appreciation of anatomical variations when they work on different specimens. Furthermore, a study suggested using innovative technology to create three-dimensional (3D) digital models of anatomical variations identified during routine dissection to teach and describe anatomical variations [ 46 ]. Thus, the 3D models will provide additional access for medical students to the identified variation along with the dissected specimen.

Other studies emphasized teaching anatomical variations in the clinical phase of medical school and postgraduate training programs. Besides learning the normal human morphology, medical students need to understand the concept of anatomical variations at the early stages of medical school and then strengthen this knowledge in the clinical years of the medical curriculum [ 47 ]. Similarly, Anscomb [ 48 ]explained the challenges to teaching and learning anatomical variation in postgraduate specialty programs. Postgraduate students should train using deep learning approaches to retain and apply the anatomical knowledge and related variations to achieve clinical best practice. Moreover, it has been found that surgical and radiological trainees learn more about anatomical variations from surgeons and radiologists than the specialty textbooks [ 10 ]. However, these variations were rarely assessed in their training curriculum. Thus, the educational leaders recommended more teaching on the anatomical variations in the early years of the resident surgical and radiological programs.

Conclusions

In summary, anatomical variation is common within clinical practice and dissection sessions. Identifying these variations is essential for medical practitioners, as it may change the clinical practice routine. Approaches for identifying anatomical variations include cadaveric dissection, physical examination, preoperative imaging assessment, and surgical procedures. Observational studies, including cadaveric dissection or medical image evaluation and evidence-based anatomy, may increase the awareness of the anatomical variation. Dissection is a common learning tool in medical schools for teaching anatomy and relevant variations. However, the literature shows a gap in educational studies evaluating the learning and assessment of anatomical variations. Thus, the current study recommends creating a health informatics database to collect common anatomical variations encountered during clinical practice to enhance awareness of these variations and improve clinical practice outcomes. Finally, there is a need to conduct more research on the learning and assessment of anatomical variations in medical curricula.

Anatomical Terminology

Before defining any specific terms, the body must be given a universal position which is known as the anatomical position. This is a universal way to position the human body so it acts as a reference point for describing anatomy. In the anatomical position the body is facing forwards, arms relaxed and by the side with the palms of the hands facing forwards.

An anatomical plane is just a slice through the body. This slice however can be done at different angles with respect to the body. The three most commonly used planes are: sagittal, coronal and transverse. The sagittal plane divides the body into left and right regions. The coronal plane separates the front (anterior) and back (posterior) halves of the body. The transverse plane demarcates the upper parts of the body from the lower e.g. it divides the torso + upper limbs from the legs + hips.

Anatomical terms of movement are used to describe ways in which the body moves due to the action of certain muscles. This movement will occur at a joint which is an articulation between at least 2 bones. The main movements in the sagittal plane are flexion and extension. In the coronal plane we have abduction and adduction. Some joints can also rotate medially (internal rotation) and laterally (external rotation). There are other movements which are specific to some joints which will be in further detail in the article.

When describing anatomical terms of location specific terms are used to avoid confusion. These terms are relative; hence the location being described must be compared to another point on the body. If there was an imaginary midline drawn on the body then anything closer towards the midline is termed medial and anything away from the midline either left or right is termed lateral e.g. the little finger lies medially when compared to the index finger. Likewise if something is further to the front of the body it is anterior and if further back it is posterior.

1.4 Anatomical Terminology

Learning objectives.

By the end of this section, you will be able to:

  • Use appropriate anatomical terminology to identify key body structures, body regions, and directions in the body

Anatomists and health care providers use terminology that can be bewildering to the uninitiated; however, the purpose of this language is not to confuse, but rather to increase precision and reduce medical errors. For example, is a scar “above the wrist” located on the forearm two or three inches away from the hand? Or is it at the base of the hand? Is it on the palm-side or back-side? By using precise anatomical terminology, we eliminate ambiguity. For example, you might say a scar “on the anterior antebrachium 3 inches proximal to the carpus”. Anatomical terms are derived from ancient Greek and Latin words. Because these languages are no longer used in everyday conversation, the meaning of their words do not change.

Anatomical terms are made up of roots, prefixes, and suffixes. The root of a term often refers to an organ, tissue, or condition, whereas the prefix or suffix often describes the root. For example, in the disorder hypertension, the prefix “hyper-” means “high” or “over,” and the root word “tension” refers to pressure, so the word “hypertension” refers to abnormally high blood pressure.

Anatomical Position

To further increase precision, anatomists standardize the way in which they view the body. Just as maps are normally oriented with north at the top, the standard body “map,” or anatomical position , is that of the body standing upright, with the feet at shoulder width and parallel, toes forward. The upper limbs are held out to each side, and the palms of the hands face forward as illustrated in Figure 1.4.1 . Using this standard position reduces confusion. It does not matter how the body being described is oriented, the terms are used as if it is in anatomical position. For example, a scar in the “anterior (front) carpal (wrist) region” would be present on the palm side of the wrist. The term “anterior” would be used even if the hand were palm down on a table.

This illustration shows an anterior and posterior view of the human body. The cranial region encompasses the upper part of the head while the facial region encompasses the lower half of the head beginning below the ears. The eyes are referred to as the ocular region. The cheeks are referred to as the buccal region. The ears are referred to as the auricle or otic region. The nose is referred to as the nasal region. The chin is referred to as the mental region. The neck is referred to as the cervical region. The trunk of the body contains, from superior to inferior, the thoracic region encompassing the chest, the mammary region encompassing each breast, the abdominal region encompassing the stomach area, the coxal region encompassing the belt line, and the pubic region encompassing the area above the genitals. The umbilicus, or naval, is located at the center of the abdomen. The pelvis and legs contain, from superior to inferior, the inguinal or groin region between the legs and the genitals, the pubic region surrounding the genitals, the femoral region encompassing the thighs, the patellar region encompassing the knee, the crural region encompassing the lower leg, the tarsal region encompassing the ankle, the pedal region encompassing the foot and the digital/phalangeal region encompassing the toes. The great toe is referred to as the hallux. The regions of the upper limbs, from superior to inferior, are the axillary region encompassing the armpit, the brachial region encompassing the upper arm, the antecubital region encompassing the front of the elbow, the antebrachial region encompassing the forearm, the carpal region encompassing the wrist, the palmar region encompassing the palm, and the digital/phalangeal region encompassing the fingers. The thumb is referred to as the pollux. The posterior view contains, from superior to inferior, the cervical region encompassing the neck, the dorsal region encompassing the upper back and the lumbar region encompassing the lower back. The regions of the back of the arms, from superior to inferior, include the cervical region encompassing the neck, acromial region encompassing the shoulder, the brachial region encompassing the upper arm, the olecranal region encompassing the back of the elbow, the antebrachial region encompasses the back of the arm, and the manual region encompassing the palm of the hand. The posterior regions of the legs, from superior to inferior, include the gluteal region encompassing the buttocks, the femoral region encompassing the thigh, the popliteus region encompassing the back of the knee, the sural region encompassing the back of the lower leg, and the plantar region encompassing the sole of the foot. Some regions are combined into larger regions. These include the trunk, which is a combination of the thoracic, mammary, abdominal, naval, and coxal regions. The cephalic region is a combination of all of the head regions. The upper limb region is a combination of all of the arm regions. The lower limb region is a combination of all of the leg regions.

A body that is lying down is described as either prone or supine. Prone describes a face-down orientation, and supine describes a face up orientation. These terms are sometimes used in describing the position of the body during specific physical examinations or surgical procedures.

Regional Terms

The human body’s numerous regions have specific terms to help increase precision (see Figure 1.4.1 ). Notice that the term “brachium” or “arm” is reserved for the “upper arm” and “antebrachium” or “forearm” is used rather than “lower arm.” Similarly, “femur” or “thigh” is correct, and “leg” or “crus” is reserved for the portion of the lower limb between the knee and the ankle. You will be able to describe the body’s regions using the terms from the figure.

Directional Terms

Certain directional anatomical terms appear throughout this and any other anatomy textbook ( Figure 1.4.2 ). These terms are essential for describing the relative locations of different body structures. For instance, an anatomist might describe one band of tissue as “inferior to” another or a physician might describe a tumor as “superficial to” a deeper body structure. Commit these terms to memory to avoid confusion when you are studying or describing the locations of particular body parts.

  • Anterior (or ventral ) describes the front or direction toward the front of the body. The toes are anterior to the foot.
  • Posterior (or dorsal ) describes the back or direction toward the back of the body. The popliteus is posterior to the patella.
  • Superior (or cranial ) describes a position above or higher than another part of the body proper. The orbits are superior to the oris.
  • Inferior (or caudal ) describes a position below or lower than another part of the body proper; near or toward the tail (in humans, the coccyx, or lowest part of the spinal column). The pelvis is inferior to the abdomen.
  • Lateral describes the side or direction toward the side of the body. The thumb (pollex) is lateral to the digits.
  • Medial describes the middle or direction toward the middle of the body. The hallux is the medial toe.
  • Proximal describes a position in a limb that is nearer to the point of attachment or the trunk of the body. The brachium is proximal to the antebrachium.
  • Distal describes a position in a limb that is farther from the point of attachment or the trunk of the body. The crus is distal to the femur.
  • Superficial describes a position closer to the surface of the body. The skin is superficial to the bones.
  • Deep describes a position farther from the surface of the body. The brain is deep to the skull.

The proximal arrow is pointing up toward the shoulder while the distal arrow is pointing down toward the hand. The lateral-medial arrow is a horizontal arrow on the abdomen. The medial arrow is pointing toward the navel while the lateral arrow is pointing away from the body to the right. Right refers to the right side of the woman’s body from her perspective while left refers to the left side of the woman’s body from her perspective.

Body Planes

A section is a two-dimensional surface of a three-dimensional structure that has been cut. Modern medical imaging devices enable clinicians to obtain “virtual sections” of living bodies. We call these scans. Body sections and scans can be correctly interpreted, only if the viewer understands the plane along which the section was made. A plane is an imaginary, two-dimensional surface that passes through the body. There are three planes commonly referred to in anatomy and medicine, as illustrated in Figure 1.4.3 .

  • The sagittal plane divides the body or an organ vertically into right and left sides. If this vertical plane runs directly down the middle of the body, it is called the midsagittal or median plane. If it divides the body into unequal right and left sides, it is called a parasagittal plane or less commonly a longitudinal section.
  • The frontal plane divides the body or an organ into an anterior (front) portion and a posterior (rear) portion. The frontal plane is often referred to as a coronal plane. (“Corona” is Latin for “crown.”)
  • The transverse (or horizontal) plane divides the body or organ horizontally into upper and lower portions. Transverse planes produce images referred to as cross sections.

The frontal or coronal plane enters through the right side of the body, passes through the body, and exits from the left side. It divides the body into front (anterior) and back (posterior) halves. The sagittal plane enters through the back and emerges through the front of the body. It divides the body into right and left halves. The transverse plane passes through the body perpendicular to the frontal and sagittal planes. This plane is a cross section which divides the body into upper and lower halves.

Body Cavities

The body maintains its internal organization by means of membranes, sheaths, and other structures that separate compartments. The main cavities of the body include the cranial, thoracic and abdominopelvic (also known as the peritoneal) cavities. The cranial bones create the cranial cavity where the brain sits. The thoracic cavity is enclosed by the rib cage and contains the lungs and the heart, which is located in the mediastinum. The diaphragm forms the floor of the thoracic cavity and separates it from the more inferior abdominopelvic/peritoneal cavity. The abdominopelvic/peritoneal cavity is the largest cavity in the body. Although no membrane physically divides the abdominopelvic cavity, it can be useful to distinguish between the abdominal cavity, (the division that houses the digestive organs), and the pelvic cavity, (the division that houses the organs of reproduction).

Abdominal Regions and Quadrants

To promote clear communication, for instance, about the location of a patient’s abdominal pain or a suspicious mass, health care providers typically divide up the cavity into either nine regions or four quadrants ( Figure 1.4.4 ).

This illustration has two parts. Part A shows the abdominopelvic regions. These regions divide the abdomen into nine squares. The upper right square is the right hypochondriac region and contains the base of the right ribs. The upper left square is the left hypochondriac region and contains the base of the left ribs. The epigastric region is the upper central square and contains the bottom edge of the liver as well as the upper areas of the stomach. The diaphragm curves like an upside down U over these three regions. The central right region is called the right lumbar region and contains the ascending colon and the right edge of the small intestines. The central square contains the transverse colon and the upper regions of the small intestines. The left lumbar region contains the left edge of the transverse colon and the left edge of the small intestine. The lower right square is the right iliac region and contains the right pelvic bones and the ascending colon. The lower left square is the left iliac region and contains the left pelvic bone and the lower left regions of the small intestine. The lower central square contains the bottom of the pubic bones, upper regions of the bladder and the lower region of the small intestine. Part B shows four abdominopelvic quadrants. The right upper quadrant (RUQ) includes the lower right ribs, right side of the liver, and right side of the transverse colon. The left upper quadrant (LUQ) includes the lower left ribs, stomach, and upper left area of the transverse colon. The right lower quadrant (RLQ) includes the right half of the small intestines, ascending colon, right pelvic bone and upper right area of the bladder. The left lower quadrant (LLQ) contains the left half of the small intestine and left pelvic bone.

The more detailed regional approach subdivides the cavity with one horizontal line immediately inferior to the ribs and one immediately superior to the pelvis, and two vertical lines drawn as if dropped from the midpoint of each clavicle (collarbone). There are nine resulting regions. The simpler quadrants approach, which is more commonly used in medicine, subdivides the cavity with one horizontal and one vertical line that intersect at the patient’s umbilicus (navel).

Chapter Review

Ancient Greek and Latin words are used to build anatomical terms. A standard reference position for mapping the body’s structures is the normal anatomical position. Regions of the body are identified using terms such as “occipital” that are more precise than common words and phrases such as “the back of the head.” Directional terms such as anterior and posterior are essential for accurately describing the relative locations of body structures. Images of the body’s interior commonly align along one of three planes: the sagittal, frontal, or transverse.

In which direction would an MRI scanner move to produce sequential images of the body in the frontal plane, and in which direction would an MRI scanner move to produce sequential images of the body in the sagittal plane?

If the body were supine or prone, the MRI scanner would move from top to bottom to produce frontal sections, which would divide the body into anterior and posterior portions, as in “cutting” a deck of cards. Again, if the body were supine or prone, to produce sagittal sections, the scanner would move from left to right or from right to left to divide the body lengthwise into left and right portions.

Humans are, of course, animals —more particularly, members of the order Primates in the subphylum Vertebrata of the phylum Chordata. Like all chordates , the human animal has a bilaterally symmetrical body that is characterized at some point during its development by a dorsal supporting rod (the notochord ), gill slits in the region of the pharynx , and a hollow dorsal nerve cord. Of these features, the first two are present only during the embryonic stage in the human; the notochord is replaced by the vertebral column, and the pharyngeal gill slits are lost completely. The dorsal nerve cord is the spinal cord in humans; it remains throughout life.

Characteristic of the vertebrate form, the human body has an internal skeleton that includes a backbone of vertebrae. Typical of mammalian structure, the human body shows such characteristics as hair , mammary glands , and highly developed sense organs.

Beyond these similarities, however, lie some profound differences. Among the mammals , only humans have a predominantly two-legged ( bipedal ) posture, a fact that has greatly modified the general mammalian body plan. (Even the kangaroo , which hops on two legs when moving rapidly, walks on four legs and uses its tail as a “third leg” when standing.) Moreover, the human brain, particularly the neocortex, is far and away the most highly developed in the animal kingdom. As intelligent as are many other mammals—such as chimpanzees and dolphins —none have achieved the intellectual status of the human species.

Chemically, the human body consists mainly of water and of organic compounds —i.e., lipids, proteins, carbohydrates, and nucleic acids. Water is found in the extracellular fluids of the body (the blood plasma , the lymph , and the interstitial fluid) and within the cells themselves. It serves as a solvent without which the chemistry of life could not take place. The human body is about 60 percent water by weight.

Lipids —chiefly fats , phospholipids , and steroids —are major structural components of the human body. Fats provide an energy reserve for the body, and fat pads also serve as insulation and shock absorbers. Phospholipids and the steroid compound cholesterol are major components of the membrane that surrounds each cell.

Proteins also serve as a major structural component of the body. Like lipids, proteins are an important constituent of the cell membrane . In addition, such extracellular materials as hair and nails are composed of protein. So also is collagen , the fibrous, elastic material that makes up much of the body’s skin, bones, tendons, and ligaments. Proteins also perform numerous functional roles in the body. Particularly important are cellular proteins called enzymes , which catalyze the chemical reactions necessary for life.

Carbohydrates are present in the human body largely as fuels, either as simple sugars circulating through the bloodstream or as glycogen , a storage compound found in the liver and the muscles. Small amounts of carbohydrates also occur in cell membranes, but, in contrast to plants and many invertebrate animals, humans have little structural carbohydrate in their bodies.

Nucleic acids make up the genetic materials of the body. Deoxyribonucleic acid ( DNA ) carries the body’s hereditary master code, the instructions according to which each cell operates. It is DNA, passed from parents to offspring, that dictates the inherited characteristics of each individual human. Ribonucleic acid ( RNA ), of which there are several types, helps carry out the instructions encoded in the DNA.

Along with water and organic compounds , the body’s constituents include various inorganic minerals. Chief among these are calcium , phosphorus , sodium , magnesium , and iron . Calcium and phosphorus, combined as calcium-phosphate crystals, form a large part of the body’s bones. Calcium is also present as ions in the blood and interstitial fluid , as is sodium. Ions of phosphorus, potassium, and magnesium, on the other hand , are abundant within the intercellular fluid. All of these ions play vital roles in the body’s metabolic processes. Iron is present mainly as part of hemoglobin , the oxygen-carrying pigment of the red blood cells . Other mineral constituents of the body, found in minute but necessary concentrations, include cobalt , copper , iodine , manganese , and zinc .

The cell is the basic living unit of the human body—indeed, of all organisms. The human body consists of trillions of cells, each capable of growth, metabolism , response to stimuli , and, with some exceptions, reproduction. Although there are some 200 different types of cells in the body, these can be grouped into four basic classes. These four basic cell types, together with their extracellular materials, form the fundamental tissues of the human body:

  • epithelial tissues, which cover the body’s surface and line the internal organs, body cavities, and passageways
  • muscle tissues, which are capable of contraction and form the body’s musculature
  • nerve tissues, which conduct electrical impulses and make up the nervous system
  • connective tissues , which are composed of widely spaced cells and large amounts of intercellular matrix and which bind together various body structures

Bone and blood are considered specialized connective tissues, in which the intercellular matrix is, respectively, hard and liquid.

How can a failure in the endocrine system affect the digestive, circulatory, and excretory systems?

The next level of organization in the body is that of the organ . An organ is a group of tissues that constitutes a distinct structural and functional unit. Thus, the heart is an organ composed of all four tissues, whose function is to pump blood throughout the body. Of course, the heart does not function in isolation; it is part of a system composed of blood and blood vessels as well. The highest level of body organization, then, is that of the organ system.

The body includes nine major organ systems, each composed of various organs and tissues that work together as a functional unit. The chief constituents and prime functions of each system are:

  • The integumentary system , composed of the skin and associated structures, protects the body from invasion by harmful microorganisms and chemicals; it also prevents water loss from the body.
  • The musculoskeletal system (also referred to separately as the muscle system and the skeletal system ), composed of the skeletal muscles and bones (with about 206 of the latter in adults), moves the body and protectively houses its internal organs.
  • The respiratory system , composed of the breathing passages, lungs, and muscles of respiration , obtains from the air the oxygen necessary for cellular metabolism; it also returns to the air the carbon dioxide that forms as a waste product of such metabolism.
  • The circulatory system , composed of the heart, blood, and blood vessels, circulates a transport fluid throughout the body, providing the cells with a steady supply of oxygen and nutrients and carrying away waste products such as carbon dioxide and toxic nitrogen compounds.
  • The digestive system , composed of the mouth, esophagus , stomach, and intestines, breaks down food into usable substances (nutrients), which are then absorbed from the blood or lymph; this system also eliminates the unusable or excess portion of the food as fecal matter.
  • The excretory system , composed of the kidneys, ureters, urinary bladder , and urethra , removes toxic nitrogen compounds and other wastes from the blood.
  • The nervous system , composed of the sensory organs, brain, spinal cord, and nerves, transmits, integrates , and analyzes sensory information and carries impulses to effect the appropriate muscular or glandular responses.
  • The endocrine system , composed of the hormone -secreting glands and tissues, provides a chemical communications network for coordinating various body processes.
  • The reproductive system , composed of the male or female sex organs, enables reproduction and thereby ensures the continuation of the species.

Phalanges of hand (Phalanges manus)

The phalanges of the hand are the group of small bones that comprise the bony core of the digits (fingers) of the hand . Even though the phalanges are small in size, they are classified as long bones because of their structural characteristics; each phalanx consists of a shaft, distal head and a proximal base.

There are fourteen phalanges in each hand; each of the medial four digits has three phalanges ( proximal , middle and distal ), while the thumb has only two ( proximal and distal ). The phalanges are interconnected by interphalangeal joints and vascularized via the nutrient rami to phalanges, which stem from the palmar digital arteries.

The digits have a universal labeling system using the anatomical position of the hand (palm facing anteriorly) as reference. Going from lateral to medial, they are named thumb (digit 1), index finger (digit 2), middle finger (digit 3), ring finger (digit 4) and little finger (digit 5) .

Proximal phalanges

Each hand has a total of five proximal phalanges , one in each digit. Compared to the middle and distal phalanges, they are the largest ones. The proximal phalanx of the thumb is an exception because it is shorter and more stout compared to the rest.

Bones of the wrist and hand

Each proximal phalanx consists of three parts;

  • The base , which represents the expanded proximal part. It has a concave, oval-shaped articular facet that articulates with the metacarpal head to form the metacarpophalangeal (MCP) joint . The base also contains nonarticular tubercles for the attachment of various soft tissue structures.
  • The body , which continues distally from the base. It tapers distally and has two surfaces; dorsal and palmar. The dorsal surface is round and smooth, appearing convex in the transverse plane. The palmar surface is flat and rough, especially on the sides where the flexor fibrous sheaths of digits attach. The surface appears flat in the transverse plane but concave in the sagittal plane.
  • The head , which represents the expanded and rounded distal part. It has a pulley-shaped articular surface that articulates with the base of the middle phalanx to form the proximal interphalangeal (PIP) joint. The heads consist of smooth grooves, especially on the palmar aspects. These grooves represent the attachment points of the collateral interphalangeal ligaments of hand.

Various ligaments attach to the proximal phalanges. The most complex one is the digital fascial complex which attaches the surrounding subcutaneous tissue and neurovasculature to the bony phalanges. The collateral and palmar metacarpophalangeal ligaments attach to the bases of the proximal phalanges. They provide strength to the metacarpophalangeal joints. The collateral interphalangeal ligaments of hand attach to the heads, supporting the PIP joints. The proximal phalanges are also covered by the extensor expansion of hand on the dorsal aspect.

The proximal phalanges are very mobile at the MCP joints . They are mainly capable of flexion, extension, adduction and abduction. Circumduction and rotation are also possible, especially at the MCP joint of the thumb. These movements are enabled by the action of several muscles;

  • Posterior (extensor) forearm muscles , such as extensor digitorum , extensor pollicis brevis , extensor digiti minimi and extensor indicis .
  • Metacarpal muscles , such as the lumbricals , palmar interossei and dorsal interossei .
  • Thenar muscles , for example flexor pollicis brevis and abductor pollicis brevis .
  • Hypothenar muscles like abductor digiti minimi and flexor digiti minimi .

These muscles carry out their functions via their direct attachments to the bases of the proximal phalanges. In addition, many extensors carry out the movements via the extensor expansion of hand which covers the phalanges.

There are four middle (intermediate) phalanges in each hand because the thumb is missing one. They have a similar structure to the proximal ones, consisting of a base, body and head. The base of each middle phalanx has two concave-shaped articular facets and matches the head of the corresponding proximal phalanx. Their apposition forms the PIP joint. The heads of the middle phalanges have a pulley-like appearance. They articulate with the bases of the distal phalanges to form the distal interphalangeal (DIP) joints of hand.

The middle phalanges are reinforced by the same ligaments supporting the proximal ones; digital fascial complex, collateral interphalangeal ligaments and extensor expansion of hand. The collateral interphalangeal ligaments attach to the base and heads of the middle phalanges to reinforce the PIP and DIP joints.

The middle phalanges are less mobile compared to the proximal phalanges. They are only capable of flexion and extension at the PIP joints . Only the flexor digitorum superficialis muscle attaches directly to the sides of the middle phalanges, flexing them at the PIP joints. The remaining contributions are provided by the action of the previously mentioned muscles; the forearm extensors, metacarpal, thenar and hypothenar muscle groups. Flexion and extension are transferred to the middle phalanges from the direct action of these muscles on the proximal phalanges or via the extensor expansion of hand.

Each hand has five distal phalanges , which look shorter and slightly thicker compared to the previous two sets. Each distal phalanx has a base, body and head. The base has a double articular facet which matches the shape of the head of the middle phalanx. The distal phalanges have a smooth and round dorsal surface. In contrast, their palmar surface is wrinkled and irregular. The nonarticular heads contain an irregular, curved shaped distal tuberosity. It serves as an anchor point for the pulps of the digits.

The distal phalanges are stabilized by the digital fascial complex, collateral interphalangeal ligaments and extensor expansion of hand. The collateral interphalangeal ligaments attach to the base of the distal phalanges to reinforce the DIP joints.

The distal phalanges are capable of flexion and extension at the DIP joints . Two forearm extensors and one flexor muscle insert directly into the bases of the distal phalanges, permitting these actions. These include flexor digitorum profundus , flexor pollicis longus and extensor pollicis longus . The previously mentioned muscle groups acting on the proximal and middle phalanges also act indirectly on the distal ones via the extensor expansion of hand.

The Difference Between Hypothesis and Theory

A hypothesis is an assumption, an idea that is proposed for the sake of argument so that it can be tested to see if it might be true.

In the scientific method, the hypothesis is constructed before any applicable research has been done, apart from a basic background review. You ask a question, read up on what has been studied before, and then form a hypothesis.

A hypothesis is usually tentative; it's an assumption or suggestion made strictly for the objective of being tested.

A theory , in contrast, is a principle that has been formed as an attempt to explain things that have already been substantiated by data. It is used in the names of a number of principles accepted in the scientific community, such as the Big Bang Theory . Because of the rigors of experimentation and control, it is understood to be more likely to be true than a hypothesis is.

In non-scientific use, however, hypothesis and theory are often used interchangeably to mean simply an idea, speculation, or hunch, with theory being the more common choice.

Since this casual use does away with the distinctions upheld by the scientific community, hypothesis and theory are prone to being wrongly interpreted even when they are encountered in scientific contexts—or at least, contexts that allude to scientific study without making the critical distinction that scientists employ when weighing hypotheses and theories.

The most common occurrence is when theory is interpreted—and sometimes even gleefully seized upon—to mean something having less truth value than other scientific principles. (The word law applies to principles so firmly established that they are almost never questioned, such as the law of gravity.)

This mistake is one of projection: since we use theory in general to mean something lightly speculated, then it's implied that scientists must be talking about the same level of uncertainty when they use theory to refer to their well-tested and reasoned principles.

The distinction has come to the forefront particularly on occasions when the content of science curricula in schools has been challenged—notably, when a school board in Georgia put stickers on textbooks stating that evolution was "a theory, not a fact, regarding the origin of living things." As Kenneth R. Miller, a cell biologist at Brown University, has said , a theory "doesn’t mean a hunch or a guess. A theory is a system of explanations that ties together a whole bunch of facts. It not only explains those facts, but predicts what you ought to find from other observations and experiments.”

While theories are never completely infallible, they form the basis of scientific reasoning because, as Miller said "to the best of our ability, we’ve tested them, and they’ve held up."

Tissue Definition

Tissues are groups of cells that have a similar structure and act together to perform a specific function. The word tissue comes from a form of an old French verb meaning “to weave”. There are four different types of tissues in animals: connective, muscle, nervous, and epithelial. In plants, tissues are divided into three types: vascular, ground, and epidermal. Groups of tissues make up organs in the body such as the brain and heart.

Types of Animal Tissues

Connective tissue connects or separates groups of other tissues. It is found in between all the other tissues and organs in the body. Connective tissue is made up of cells and ground substance, which is a gel that surrounds cells. Most connective tissue, except for lymph and blood, also contains fibers, which are long, narrow proteins. Fibers can be collagenous, which bind bones to tissues; elastic, which allow organs like the lungs to move; or reticular, which provide physical support to cells. Connective tissue also allows oxygen to diffuse from blood vessels into cells.

About 1 in 10 people are have a disorder involving connective tissue. Some connective tissue disorders include sarcomas, Marfan syndrome, lupus, and scurvy, which is a Vitamin C deficiency that leads to fragile connective tissue.

Muscle tissue comprises all the muscles in the body, and the specialized nature of the tissue is what allows muscles to contract. There are three types of muscle tissue: skeletal muscle, cardiac muscle, and smooth muscle. Skeletal muscle anchors tendons to bones and allows the body to move. Cardiac muscle is found in the heart and contracts to pump blood. Smooth muscle is found in the intestines, where it helps move food through the digestive tract, and it is also found in other organs like blood vessels, the uterus, and the bladder. Skeletal and cardiac muscles are striated; this means that they contain sarcomeres (a unit of muscle tissue) that are arranged in a uniform pattern. Smooth muscle does not have sarcomeres.

Duchenne muscular dystrophy is an example of a muscle tissue disorder. It is an inherited disorder that causes muscles to atrophy over time. The muscles shorten as they atrophy, which can cause scoliosis and immobile joints. Individuals with the disorder are usually male because the gene responsible for it is found on the X chromosome (of which males have only one).

Nervous tissue is found in the brain, spinal cord, and peripheral nerves, which are all parts of the nervous system. It is made up of neurons, which are nerve cells, and neuroglia, which are cells that help nerve impulses travel. Nervous tissue is grouped into four types: gray matter and white matter in the brain, and nerves and ganglia in the peripheral nervous system. The main difference between gray and white matter is that axons of the neurons in gray matter are unmyelinated, while white matter is myelinated. Myelin is a white, fatty substance that insulates neurons and is crucial for nervous system functioning.

The symptoms of Alzheimer’s disease, such as memory loss, mood swings, and confusion, are caused by the breakdown of nervous tissue. Amyotrophic Lateral Sclerosis (ALS) is another disease that causes nervous tissue to degenerate, which leads to the loss of higher brain functions over time. Other nervous tissue disorders include multiple sclerosis, where the immune system attacks and destroys nervous tissue, Huntington’s disease, where an abnormal protein causes neuron death, and Parkinson’s disease, where the part of the brain that controls movement is impaired because not enough dopamine is produced.

Epithelial tissue, or epithelium, covers the surfaces of organs including the skin, the trachea, the reproductive tract, and the digestive tract’s inner lining. It creates a barrier that helps protect organs, and it also has roles in absorbing water and nutrients, getting rid of waste, and secreting enzymes or hormones. All of the body’s glands are formed from ingrowths of epithelium. Some common epithelial tissue diseases are skin diseases like eczema and psoriasis, which both cause rashes. When cancer develops from epithelial tissue, it is called a carcinoma. Epithelial cells in the airways are also responsible for asthma, which is characterized by inflammation of the airways that leads to shortness of breath.

Types of Plant Tissues

Vascular tissues in plants transport substances throughout the different parts of the plant. The two types of vascular tissue are xylem and phloem. Xylem transports water and some soluble nutrients, while phloem transports organic compounds the plant uses as food, particularly sucrose. Vascular tissues are long and thin, and form cylinders that nutrients are transported through like pipes. Vascular tissue is also involved with two types of meristems, which are tissues that contain undifferentiated cells that are used during a plant’s growth. The meristems accompanying vascular tissue are the cork cambium and the vascular cambium. These meristems are associated with the growth of the plant’s vascular tissues.

Ground tissue is made up of all cells that are not vascular or dermal (having to do with the epidermis; see below). There are three types of ground tissue: parenchyma, collenchyma, and sclerenchyma. Parenchyma cells form the “filler” tissue in plants, and perform many functions like photosynthesis, storage of starch, fats, oils, proteins, and water, and repairing damaged tissue. Collenchyma tissue is made up of long cells with irregularly thick walls that provide structural support to the plant. Plants that grow in windy areas have thicker walls of collenchyma tissue. Sclerenchyma is also supporting tissue, but it is made of dead cells. There are two types of sclerenchyma: fibers and sclereids. Fibers are long, slender cells, while sclereids are star-shaped with thick cell walls. Sclerenchyma fibers make up fabrics such as hemp and flax.

The epidermis is made up of a single layer of cells that covers a plant’s roots, stems, leaves, and flowers. (Epidermis is also the word for skin in human anatomy.) It guards the plant against water loss, regulates the exchange of carbon dioxide and oxygen, and in roots, it absorbs water and nutrients from the soil. The epidermis on a plant’s stems and leaves have pores called stomata, which carbon dioxide, water vapor, and oxygen diffuse through. Epidermal cells are themselves covered by the plant cuticle, which contains mainly cutin, a waxy substance that protects against water loss. Plants in deserts and other arid regions often have thick cuticles to help conserve water.
