The Plan-Do-Study-Act (PDSA) Worksheet is a useful quality improvement tool for documenting a test of change. The PDSA cycle, an essential component of the Model for Improvement , is shorthand for testing a change by developing a plan to test the change (Plan), carrying out the test (Do), observing and learning from the consequences (Study), and determining what modifications should be made to the test (Act).
Use the Plan-Do-Study-Act (PDSA) Worksheet to help your team document a test of change. Fill out one PDSA Worksheet for each test you conduct. Your team will test several different changes, and each change will go through several PDSA cycles. Keep a file (either electronic or hard copy) of all PDSA Worksheets for all changes your team tests.
*NOTE: Before filling out the template, first save the PDF file to your computer. Then open and use that version of the tool. Otherwise, your changes will not be saved.

The PDSA Cycle (Plan-Do-Study-Act) is a systematic process for gaining valuable learning and knowledge for the continual improvement of a product, process, or service.
The Plan-Do-Study-Act Cycle Groups commonly use the Plan-Do-Study-Act (PDSA) cycle in continuous improvement processes to formalize an investigation of the model for improvement—the three questions listed above.

PDSA, or Plan-Do-Study-Act, is an iterative, four-stage problem-solving model used for improving a process or carrying out a change. In applying PDSA, ask yourself three questions: 1. GOAL: What are we trying to accomplish? 2. What change(s) can we make that will result in improvements? 3. How will we know the change is an improvement?
PLAN When starting a PDSA cycle, the first step is to plan the cycle’ s details. The details should include what will be changed or tested, who should be involved, where information will be collected, and what data are needed to determine whether goals are met.
